Health Clarity Assessment – Personalised Nutrition & Functional Medicine Review
When you’re dealing with ongoing health concerns, it can be difficult to know what deserves your attention. You may have tried different supplements, made dietary changes, received laboratory results or researched possible explanations for your symptoms, yet still feel unsure about what to do next.
The Health Clarity Assessment helps you make sense of your current health situation, understand what may be relevant and identify your most useful next steps. It combines a structured review of your health history with a 30-minute online consultation and a personalised written Health Priorities Plan.
Rather than adding more information to sort through, the aim is to help you understand what matters, what remains uncertain and where to focus your efforts. Your assessment also includes 14 days of follow-up support to clarify your plan and recommendations.
Fairfield Nutrition Thoughts
When this assessment is a good choice
This assessment is particularly useful when you’ve been trying to improve your health but aren’t sure whether you’re focusing on the right things. Perhaps you’ve tried several approaches without much progress, received conflicting advice or want an experienced practitioner to review your current supplements, diet and health information.
The aim is to give you greater clarity about your health, identify your most useful priorities and provide practical direction for what to do next. You’ll receive a personalised written plan and 14 days of follow-up support to clarify your recommendations.
When it may not be the right choice
The Health Clarity Assessment is a focused review designed to provide clarity and direction, rather than a comprehensive investigation or ongoing health management programme.
- If you need detailed interpretation of multiple laboratory reports, a dedicated results consultation may be more appropriate.
- If you’re looking for ongoing investigation, regular reviews and adjustments to your health plan, our Health Unlimited programme may be a better fit.
- If you have severe, sudden or worsening symptoms, seek appropriate medical attention rather than waiting for an assessment.
- The assessment doesn’t provide a medical diagnosis or replace care from your doctor or other healthcare professionals.
The assessment is a complete, standalone service. The goal is to help you make informed decisions about your next steps, whether you continue independently, with Fairfield Nutrition or with your existing healthcare practitioners.
Our Functional Medicine Approach
Rather than considering individual symptoms in isolation, we look at your broader health history, nutrition, lifestyle and the circumstances surrounding the development of your concerns.
The assessment draws on functional medicine frameworks, including the Functional Medicine Timeline and Matrix, to help organise relevant information, explore possible contributing factors and identify areas that deserve attention.
What we consider
- Your health history: When symptoms began, how they’ve changed and significant events that may be relevant.
- Potential contributing factors: Dietary, lifestyle, environmental and other influences worth exploring.
- Connections between systems: How different aspects of your health may relate to one another.
- Your current approach: Whether existing supplements, dietary strategies and investigations are appropriate for your circumstances.
- Your priorities: Which questions or practical steps are most useful to address first.
The aim isn’t to assume that every symptom is connected or that there’s always a single underlying cause. It’s to consider the bigger picture while distinguishing established findings from possibilities that may require further assessment.
The Timeline and Matrix are frameworks for organising and evaluating health information, not diagnostic tests. We use them to inform the assessment rather than automatically recommending more testing or supplements.
How It Works

Share your history → Practitioner review → Online consultation → Your personalised plan
1. Complete your health questionnaire
After purchasing your assessment, you’ll receive instructions for completing your online health questionnaire and booking your consultation.
The questionnaire covers your main concerns, symptoms, health history, medications, supplements, lifestyle and relevant previous investigations.
Please complete it at least two business days before your appointment to allow time for preparation.
2. Professional review of your information
Stephen Ward reviews your questionnaire and relevant supporting information before your appointment.
Using a structured functional medicine approach, he considers your health history, potential contributing factors, areas of uncertainty and which questions deserve attention.
3. Your 30-minute online consultation
Meet with Stephen for a focused discussion of your health concerns, what you’ve already tried and what may be worth prioritising.
You’ll have the opportunity to ask questions, discuss relevant observations and explore reasonable next steps.
4. Receive your personalised Health Priorities Plan
Within two business days of your consultation, you’ll receive a written plan summarising the key observations, priorities and recommendations from your assessment.
Your plan provides a practical reference you can return to as you consider your next steps.
What You Will Receive
At the centre of your assessment is a personalised written Health Priorities Plan. Rather than a lengthy list of recommendations, it focuses on the findings and decisions most relevant to your circumstances.
Your Health Priorities Plan
- Key observations: What stands out from your health history, current symptoms and existing approach.
- Areas of uncertainty: Important questions, information gaps or possible explanations that may need further consideration.
- Your top priorities: Up to three areas that deserve attention first, so you’re not trying to address everything at once.
- Recommended next steps: Practical considerations relating to nutrition, lifestyle, supplements or further assessment, where appropriate.
- When to reassess: What to monitor, when to review your progress and when additional professional or medical support may be worthwhile.
Everything included in your assessment
- Comprehensive online health questionnaire: An opportunity to provide your history, symptoms, current approach and relevant health information before your appointment.
- Professional practitioner review: Stephen evaluates your information using a structured, functional medicine-informed approach.
- 30-minute online consultation: A focused discussion of your concerns, observations and priorities.
- Personalised written Health Priorities Plan: Your key observations, uncertainties, priorities and recommended next steps, delivered within two business days of your consultation.
- 14 days of follow-up support: An opportunity to clarify anything in your assessment or written plan.
The assessment is a complete, standalone service. You don’t need to enrol in another programme to receive your written recommendations.
Follow-Up Support
It’s common to think of additional questions after a consultation or while reading through your written recommendations.
That’s why your Health Clarity Assessment includes 14 days of follow-up support, starting when your Health Priorities Plan is delivered.
What’s included?
- Clarification of findings or explanations in your written plan.
- Questions about your suggested priorities and next steps.
- Help understanding how to begin implementing the recommendations discussed during your assessment.
You can send questions electronically, and we aim to respond within two business days.
What’s not included?
- Assessment of new symptoms or unrelated health concerns.
- Interpretation of new laboratory results.
- Substantial changes to your original recommendations or development of new protocols.
- Ongoing monitoring or clinical case management.
If further assessment is needed, we’ll explain the options rather than automatically recommending another consultation.

Common Questions
What does the assessment price include?
Your purchase includes the online health questionnaire, professional review of your information, a 30-minute online consultation with Stephen Ward, your personalised written Health Priorities Plan and 14 days of follow-up clarification support. It’s a one-off service with no ongoing commitment.
What is the introductory price?
We’re offering the Health Clarity Assessment for $195 for the first 10 assessments as we introduce the service. The proposed standard price is $295. The full assessment and follow-up support are included at the introductory price.
How is this different from a regular nutrition consultation?
The assessment includes structured preparation using functional medicine principles, a focused consultation and a written Health Priorities Plan. The emphasis is on identifying what matters most and making appropriate next-step decisions, rather than providing general advice during a consultation alone.
Do I need laboratory tests before booking?
No. We can review your existing health information and consider whether any further testing is likely to be useful. Additional tests aren’t automatically recommended, and any tests purchased separately involve an additional cost.
Can you review my existing laboratory results?
Relevant existing results can be considered as part of your health history. However, extensive interpretation of multiple laboratory reports isn’t included in this focused assessment and may require a separate service.
How long does the assessment take?
You’ll complete your online questionnaire before your appointment. Stephen reviews your information in advance, and your online consultation lasts 30 minutes. Your written Health Priorities Plan follows within two business days.
What if I have questions after receiving my plan?
You have 14 days from delivery of your Health Priorities Plan to send questions clarifying your assessment and recommendations. We aim to respond within two business days. New concerns or substantial changes to the plan may require further assessment.
Will I need ongoing consultations?
Not necessarily. The assessment is designed to provide useful direction in its own right. If you would benefit from more extensive investigation or ongoing support, we can discuss further consultations or Health Unlimited, but there’s no obligation to continue.
Is this a medical diagnosis?
No. This is a focused nutrition and health assessment, not a substitute for medical diagnosis or treatment. Where concerns require medical investigation or management, we’ll recommend an appropriately qualified healthcare professional.
